This document is a list containing all the items needed for your home birth. All of these items should be collected before your 36 week home visit.

Initial Labwork Informed Consent

Group B Streptococcus (GBS)

Vitamin K Injection for the Newborn

Informed consent providing information about the synthesis of vitamin k in the newborn. This document explains why giving a Vitamin K injection to newborns has become the standard of care.
